To get it back you can either download again from app store (you won't get charged if you've already bought it) or do it through iTunes. In iTunes with your phone attached select your device and then click on the applications tab. It sounds to me like you’ve caught the x to delete the icon when dragging the icon across pages, this is a easy easy thing to do especially when trying to get the damn page to flip while holding the icon.
